This role is for our UJIMA Program location in Harlem. This program is a residential setting for individuals facing substance abuse.
SCOPE OF THE ROLE:
Under the supervision of the program director, the Licensed Practical Nurse coordinates the medical management of participants’ issues to optimize health.
ESSENTIAL DUTIES & RESPONSIBILITIES:

- Provide habilitation services to each client as prescribed in that client’s service plans (CFA, Individual Service plan, Habilitation plan)
- Assist each client, as needed, in skill development in areas such as activities of daily living, domestic living, recreation, socialization.
- Ensure that each assigned client is well-groomed, including hairstyle, finger and toenail trimming, good oral and personal hygiene.
- Ensure that each assigned client’s living area is clean and tidy.
Location: Brooklyn, NY
Schedule: Monday - Friday 4 days 9 am - 5pm, 1 late day 11 am - 7pm, 1 saturday per month.
